3 ‘never events’ already in one Hospital Trust this year

More news today about how a patient received an intravitreal injection into the wrong eye which follows not long after another patient undergoing a total knee replacement on the wrong knee.

‘Never events’ are errors that should never happen and that are entirely preventable.
North Cumbria University Hospitals NHS Trust board received papers revealing two ‘never events’ in quick succession, one in November and another in December.

Worse news still is that it was revealed that there have been three never events in January and February of this year already. One relates to a swab being left inside a patient from an operation in June last year and there is no information available yet relating to the other two ‘never events’.

The new chief executive Stephen Eames has instigated an independent review of surgical procedures within the trust.
